I tried on a fresh install of Omeka 2.1.4 and OAI-PMH-Harvester-2.0. And tried on a few repositories, like
http://lawspace.stmarytx.edu/oai-pmh-repository/request
http://dspace.willardlibrary.org/oai/request
All result as Queued.
No apache error, from mysql log, I can see INSERT INTO omeka_oaipmh_harvester_harvests and INSERT INTO omeka_oaipmh_harvester_harvests, but no records added.
I also tried on a fresh install on Omeka 1.5 and Harvest 1.0, and all result as Starting.

BackgroundProcess is not allowed, so
jobs.dispatcher.longRunning = "Omeka_Job_Dispatcher_Adapter_Synchronous"

I see 5 metadata available in the repository, cdwalite, mets, mods, oaidc and omekaxml.
But I only see 2 options in harvest, mets and oaidc?
